بررسی فراوانی ایزوفرمهای ژن APOE و ارتباط آن با برخی فاکتورهای پاراکلینیکی در میان سالمندان مبتلا به فراموشی ناشی از افزایش سن در منطقه سیس

درجهای از دمانس یا فراموشی در بسیاری از موجودات زنده ازجمله انسان همراه با افزایش سن و پیر شدن ایجاد میشود. اینگونه اختلالات ممکن است ناشی از عوامل ژنتیکی و بالینی متفاوتی باشد. در این پژوهش تعیین پلیمورفیسم ژن APOE بهعنوان مهمترین فاکتور ژنتیکی شناختهشده برای بیماری آلزایمر، در گروهی از سالمندان مبتلا به فراموشی در منطقه سیستان صورت گرفت و ارتباط آن با وضعیت چربی خون، سطح یونهای فلزی خون و برخی ویژگیهای دموگرافیک مرتبط با سبک زندگی بررسی شد. همچنین در پژوهش حاضر، ارتباط بین عوامل محیطی مربوط به سبک زندگی و فاکتور ژنتیکی APOE مورد بررسی قرار گرفت. با اخذ مجوزهای قانونی و گرفتن رضایتنامه از افراد شرکتکننده در مطالعه، پس از غربالگری افراد جمعیت هدف با استفاده از پرسشنامه استاندارد (MMSE) Mini Mental Estate Examination و همچنین در نظر گرفتن نژاد سیستانی از گروه سالمندان بیمار و سالمندان سالم بهعنوان شاهد نمونه خون تام تهیه شد. نمونهها شامل 55 فرد بیمار و 70 فرد سالم بود. افراد سالم انتخاب شده از نظر سن، جنس و نژاد با گروه بیمار همخوان بودند. از سرم خون آنها جهت سنجش میزان چربی و میزان عناصر فلزی مس، آهن و روی استفاده شد. استخراج DNA از گلبولهای سفید خون محیطی انجام گرفت و بررسی ژنوتیپ افراد برای ژن APOE با روش (AS PCR) allele specific PCR صورت پذیرفت. پارامترهای دموگرافیک شامل سن، جنسیت، سطح سواد، مصرف دخانیات و مواد مخدر، وجود سابقه خانوادگی برای بیماری و سابقه بیماریهای قلبی و صدمه سر نیز برای هر یک از افراد بیمار و همینطور گروه شاهد مورد ارزیابی قرار گرفت. نتایج بیانگر وجود فراوانی بیشتر آلل E4 ژن APOE در میان سالمندان بیمار نسبت به سالمندان سالم بود. شایان ذکر است که میزان HDL خون سالمندان بیمار کمتر از افراد سالم بود درحالیکه سطح فلز روی در خون گروه شاهد کمتر از گروه بیماران بود. بهطورکلی فراوانی زنان مبتلا به فراموشی بیشتر از مردان بود. همچنین میانگین سطح سواد در بیماران بسیار کمتر از گروه شاهد بود درحالیکه مصرف مواد مخدر در بین بیماران شایعتر از سالمندان سالم بود. در دیگر فاکتورهای بررسی شده تفاوت معناداری میان دو گروه مورد مطالعه مشاهده نشد.
A degree of age-related dementia has been well documented in almost all eukaryotes including human being. Symptomatic stages of dementia are manifested at later ages of life. There are several environmental and genetic factors leading to neuronal death in elderly individuals. In the present study, for the first time in Sistan Baluchestan province, status of the environmental factors related to lifestyle and their association with a known genetic risk factor for Alzheimer’s disease, APOE, has been investigated. There were 55 affected and 70 healthy individuals participated in our study. Controls were age, sex, and ethnicity-matched with cases. This study was performed according to local institutional human ethical criteria and an informed written consent was obtained from all the participants. After screening of target population according to standard MMSE test, peripheral blood samples were drawn and used to determine lipid profile and metal ion levels of each individual. The DNA samples were also isolated from peripheral blood samples and underwent allele specific PCR (AS PCR) for APOE genotyping. Demographic factors such as age, sex, education level, smoking and substance addiction, heart attack and brain injury experience, and family history of disease were recorded. Our results showed a noticeable increase in allelic frequency of APOE E4 among cases. The blood HDL level of cases was markedly lower than that of controls while Cu ion has a higher level in peripheral blood samples of cases when compared to healthy control individuals. Moreover, most of the affected people were illiterate and drug abuser. Other assessed paraclinical factors displayed no significant differences between the two groups.
